Diagnosis: Non-healing erosions etiology unclear. One always considers factitial disease in health care professionals with atypical skin lesions and this man also has free access to needles as a diabetic. In a year, the scalp lesion has shown no tendency to improve.
Further Treatment: He was treated with topical corticosteroids in case this was erosive pustular dermatitis of the scalp (no response) and imiquimod in case erosion might have been hypergranulation tissue. (no response) We ordered Duoderm dressings, but they were too expensive for the patient.
Questions: Where would you go from here? Diagnostic and therapeutic suggestions.">Stumped...

Unlike Jeff and me, Peabody's house hunting days are over. She and her husband have a lovely new home, and to celebrate, Peabody is hosting a housewarming party. I'm bringing her homemade Sweet Potato Gnocchi with Maple Cinnamon Sage Brown Butter. I know, that's the longest name ever, but it's not mine, it's Giada's.
When my parents were here for Thanksgiving, they gushed so much about Giada's sweet potato gnocchi that I surprised my dad and made them for his birthday. Honestly, they were delicious. Made with baked sweet potatoes and creamy fresh ricotta cheese, these gnocchi are pillowy and comforting. I especially love the way the fragrant, sweet, maple sauce clings to the grooves.
Here is Giada's recipe for Sweet Potato Gnocchi from her show Everyday Italian. I made the following changes:
I substituted Garnet yams for sweet potatoes. I used pretty purple sage instead of green.I used about half the butter Giada called for, but that's up to you. I am submitting this recipe to Glenna of A Fridge Full of Food, this monthâ??s host for Weekend Breakfast Blogging. She has chosen an excellent theme: ethnic dishes with a twist. This recipe is adapted from Nigella Lawsonâ??s cookbook Forever Summer.
Ricotta Hotcakes

Nigella says this recipe makes about 25 hotcakes. That's if you like miniature ones. I made about 12 "American sized" hotcakes.
9 ounces (1 cup plus 2 Tbsp ricotta cheese)
œ cup low fat milk
2 large eggs, separated
Ÿ-1 cup all-purpose flour (add more to make batter thicker if necessary)
1 tsp baking powder
Pinch salt
Canola oil for the cooking the cakes o the griddle
Add ricotta, milk, and egg yolks into a bowl and mix well to combine. Stir in the flour, baking powder, and salt, and gently whisk to make a smooth batter.
In a glass or metal bowl, whisk egg whites until they become foamy, then fold them into the ricotta mixture.
Heat the oil on a griddle or large frying pan. Pour batter onto the griddle. Cook for about 1 minute until golden, then flip and cook another minute. Keep the cooked hotcakes warm by tenting them with foil on a large warmed plate, while you make the rest.
Serve with honeyed peaches and nectarines below, or with a topping of your choice, such as maple syrup, fresh berries, or wild honey.
Peaches and Nectarines in a Honey Syrup
Ÿ cup water
1/8 cup good honey
2 Tbsp sugar
A few dashes each of ground cinnamon and nutmeg
Œ tsp pure vanilla extract
1 large ripe peach, sliced
1 large rip nectarine, sliced
Combine the water, honey , and sugar in a pot. Bring to a boil, stirring until the sugar dissolves. Add the cinnamon, nutmeg, and vanilla. Continue to simmer for 3-5 minutes. Add the peaches and nectarines, lower heat to a simmer, and cook for about 6-8 minutes, until fruit is soft but not mushy. The syrup should start to reduce and thicken a bit. Allow to cool slightly before adding to the hotcakes.
When ready, place hotcakes on a plate, layering with cooking fruit and drizzling with more of the honey syrup. Garnish with a couple of slices of ripe but uncooked fruit slices, if desired.

"Rubbery" is never a good thing to say about muffins.
This was not to be my last baking difficulty. Baking requires precision; forgetting one ingredient such as baking soda can completely ruin a dish. Trust me.
My mom, unlike me, has managed to become an extraordinary baker. She even modified her recipes to accommodate the no-fat days of the '90s and the no-carb days of this century without ever sacrificing flavor.
When Joanna of Joanna's Foods announced low-fat baked goods as this month's topic for Heart of the Matter, a wonderful heart healthy food event which she co-hosts with Ilva of Lucullian Delights, I thought I'd give low-fat baking another try. But first, I called my mom. I asked her if she had a recipe for a baked good that had no butter or cream and very little margarine, oil, and egg yolks; she suggested scones.
I noted it did not include apple sauce. Instead, non-fat cottage cheese and buttermilk (which is lower in fat than regular milk) keep the scones moist. I told her I had some fresh, sweet medjool dates I wanted to use. We started brainstorming and came up with a dynamic combination of flavors: dates, fennel seeds, pistachios, and orange zest.
Mom assured me that scones are about â??the easiest things you can bake.â? They really were easy; the hardest part was having to refer to the recipe 27 times to make sure I didnâ??t miss anything. I heeded her warnings, most especially the one about not over mixing the batter or itâ??ll become "like lead."
